Students new to the OCDSB who wish to register at South Carleton must complete the following forms:
Student Information

Appropriate Use of Technology and Media Permissions
Please make an appointment by phoning the Office Administrator at 613-838-2212 ext. 105. Bring these documents with you to the appointment:
proof of address (property tax bill, assessment notice, purchase of sale agreement, or rental agreement); and
a minimum of one other form of identification that includes the address and the guardian’s name (which may include the previous list as well as utility bills, driver’s license, etc.); and
copy of Canadian Birth Certificate, Passport, or Permanent Resident documentation.

Certain courses require that additional forms be completed in order for a student to register. Instructions and application forms for those courses may be downloaded below. These must be submitted to Guidance.
Register for your courses using Career Cruising. Read the instructions! Registration will begin on Friday, February 10.
Once course selection is completed, print the Sign Off sheet (3 pages). This must be signed by a parent and handed in to Guidance by Wednesday, February 22. 2017.

Grade 12 Students
Any grade 12 student wishing to return to South Carleton for a victory lap must complete this form and return it to Guidance by February 22, 2017.
